Business information we collect
When you request clinic services, we collect the clinic name, Florida city, optional AHCA license number, contact name, work email, phone, preferred language and preferred callback time. We use these details to review your request and coordinate onboarding.
Hosting, forms and clinic sign-in
The public website and its clinic-request form are hosted by Netlify. The protected clinic portal is a separate ChatGPT Site. Authorized clinic staff sign in to the portal with their own account and can access only the clinic assigned to them. Hosting and authentication providers process information under their own terms. Website fonts may be requested from Google Fonts.
Patient information stays out of the public website
Do not submit patient names, identifiers, injury details, records or insurance documents through the public website or business email. Authorized clinics use the protected portal and the workflow agreed with the physician.
Technical information and external links
Hosting providers may process technical access information to operate and protect their services. Doxy.me, calendar and other external links open separate services governed by their own privacy terms.
